Dental care is one of the benefits under the Hawaii Medicaid program administered by
the Med-QUEST Division (MQD) of the Department of Human Services. Community
Case Management Corp (CCMC) is delegated authority, through a contract with the
Hawaii Dental Service (HDS), to help Medicaid-eligible individuals access dental care.
We provide assistance to Medicaid-eligible individuals on all of the islands. We will help
you to find a dentist who can provide the covered dental benefits that you need.
- Children and adolescents under 21 yrs of age: Diagnostic and preventive
services are covered. Services include X-rays, cleanings, preventive care such
as topical fluoride and restorative care such as fillings for cavities. - Adults 21 years and older: Selected preventive and restorative dental
procedures are covered. Limited denture benefits are also covered. Cosmetic
orthodontia (braces) is not covered.
Your dentist will determine the covered benefit that is appropriate to your dental needs.

We will provide you with the name and contact information of an appropriate Medicaid
dental provider on your island. You will be asked to contact the dental provider and to
schedule your own appointment.
Once you have made an appointment:
- Be sure to keep your appointment and arrive on time;
- Bring your Medicaid ID card and a picture identification to the dental
office, - If you don’t have a Medicaid ID card, call MQD at 1-800-316-8005 to
request one; and
- Bring your Medicaid ID card and a picture identification to the dental
- If you need to cancel your appointment, please call the dental office at least 24 hours in advance of your appointment.
